Overview
Heathwood Assisted Living at Penfield is seeking a full-time Assistant Case Manager to assist our current full-time Case Manager. Duties to include assisting with pre-admission screening, admissions, discharge planning and helping residents and family members with healthcare related concerns in conjunction with the nursing department. Available position is 20 hours per week, primarily days although occasional evenings and weekends may be required. In addition to 20 hours a week in Case Manager, this position will be managing our reception department by creating schedules and filling in at the reception desk when needed.

- Ability to read and understand task assignments, ability to report verbally and provide written documentation as required

Required Experience:
Qualifications
- High School Diploma/GED required.
- Training in Social Work or related field preferred.
- Valid New York Drivers License and clean driving record required.
- Knowledge in Microsoft Office programs preferred.
- Case work experience preferred.
- Ability to meet the public easily, handle problems diplomatically, and cooperate with other staff members necessary.
- Evidence of professional growth, continuing education and community involvement considered.
- Ability/Willingness to work closely with Case Manager, Resident Care Manager and other healthcare team members.
