    Job Description

    The part-time Case Manager provides case management and student support services to adult education students, helping individuals identify and overcome barriers to education, employment, and personal success. Assist students with enrollment, orientation, academic and career planning, referrals to campus and community resources, and ongoing progress monitoring. Build supportive relationships with students, maintain accurate documentation, coordinate with faculty and program staff, and connect students with appropriate resources to promote persistence, completion, and successful transition to employment, postsecondary education, or career training.

    This position is part-time, up to 19 hours/week.

    DUTIES AND RESPONSIBILITIES:

    Data Management and Administrative Support:

    • Maintain accurate and up-to-date student records, case notes, and program documentation.

    • Enter and update student information in CRM, student information systems, and other program databases.

    • Track student enrollment, attendance, participation, academic progress, and program completion.

    • Monitor student caseloads and maintain organized records of case management activities and follow-up.

    • Prepare and maintain spreadsheets, reports, rosters, tracking logs, and other administrative documents.

    • Assist with data collection and reporting requirements for Adult Education programs, grants, and funding sources.

    Program Support:

    • Provide direct support to Adult Education students throughout their educational journey.

    • Assist with student recruitment, intake, enrollment, orientation, and onboarding activities.

    • Support students in developing educational, career, and personal goals.

    • Monitor student participation, attendance, progress, and program persistence.

    • Conduct regular outreach and follow-up with students to encourage engagement and successful program completion.

    • Connect students with academic, career, workforce, and community resources based on individual needs.

    • Assist with coordinating student workshops, events, information sessions, and other program activities.

    • Collaborate with instructors, advisors, staff, and community partners to support student success.

    • Provide referrals for supportive services, including transportation, childcare, employment, housing, and other community resources, as appropriate.

    • Help identify barriers that may affect student participation and assist in developing strategies to address those barriers.

    • Support students in transitioning to postsecondary education, workforce training, credential programs, or employment opportunities.

    • Participate in program meetings, staff trainings, and professional development activities.

    • Assist with outreach and communication efforts to promote Adult Education programs and services.

    • Support special projects and initiatives that enhance student engagement, retention, and completion.

    • Provide general program and operational support to the Adult Education team as needed.

    Student Support:

    • Student Assessment: Meet with students to assess their educational, academic, career, and personal support needs and identify barriers that may impact their success.

    • Individual Success Planning: Work with students to develop realistic, step-by-step education and career goals, including action plans for enrollment, persistence, and program completion.

    • Resource Coordination: Connect students with appropriate CCRI resources and community services, including academic support, career services, workforce training, financial resources, transportation, childcare, and other community-based assistance.

    • Progress Monitoring: Conduct regular check-ins with students to monitor attendance, academic progress, engagement, and progress toward their individual educational and career goals.

    • Student Advocacy and Support: Advocate for students by helping them navigate college processes, address barriers, communicate with appropriate departments, and access available academic and support services.

    • Documentation and Data Management: Maintain accurate and confidential records of student contacts, appointments, referrals, case notes, progress updates, and support services in appropriate college databases and digital systems.

    • Student Crisis and Barrier Support: Assist students in addressing urgent barriers that may interfere with their educational participation and connect them with appropriate CCRI departments, community resources, or emergency services when necessary.

    REQUIRED QUALIFICATIONS:

    • Undergraduate or Graduate Degree: Bachelor's degree in Social Work (BSW), Psychology, Nursing, or Human Services; some residential or entry-level roles accept a High School diploma with relevant experience.

    • Field Experience: 1 to 3 years of supervised experience in behavioral health, substance use treatment, or social services.

    • Experience working with adult students

    • Demonstrated work experience providing outstanding customer service

    • Demonstrated excellent time management skills, high learning agility, creative problem solving, interpersonal, oral and written communications, and presentation skills, with strong attention to detail and accuracy

    • Demonstrate proficiency with customer relationships management or data management enterprise software

    • Demonstrated proficient computer skills including Microsoft Office (Word, Excel, and PowerPoint, primarily), Google G-Suite and email, Internet applications, and database software

    • Experience supporting students from diverse backgrounds to engage in education

    • Demonstrated knowledge and understanding of both assessment and college and/or career readiness curricula for adult students

    • Knowledge of workforce development services, programs, and /or program providers.

    • Bilingual skills-Spanish preferred

    • Knowledge of social and support services available in the community
