Minimum Qualifications

Who We Are:

Mental Health Support Services (MHSS) is the provider of mental health, developmental disabilities, and substance use disorder services for children and adults in Chesterfield County. The Assertive Community Treatment (ACT) team is seeking a qualified and experienced Case Manager to join our dynamic, interdisciplinary team that provides intensive, community-based services to individuals with serious mental illness diagnoses.

Minimum Education and Experience:

Bachelors degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field; two years of experience in a human services field; or an equivalent combination of training and experience. Two years of experience working with individuals with mental illness preferred; experience with benefits navigation, Department of Social Services/Social Security Administration systems, or case management preferred. Qualified Mental Health Professional certification preferred; experience working for a Community Services Board (CSB) preferred; experience with an electronic health record preferred.

Required Knowledge, Skills, and Abilities:

Working knowledge of case management methods, practices, and procedures. Ability to interview clients to identify deficiencies in personal, social, economic and health needs; to work in a multi-disciplinary team environment providing treatment and rehabilitation services; to communicate effectively orally and in writing; to operate a personal computer and related software and other standard office equipment; to develop and maintain effective working relationships with internal and external customers. Knowledge of mental illness, community resources, Medicaid benefits, Supplemental Nutrition Assistance Program (SNAP), housing resources, and local support systems preferred. Strong organizational and time management skills for tracking deadlines, benefits cycles, and documentation requirements preferred. Ability to work effectively with clients experiencing homelessness, acute mental health symptoms, co-occurring disorders, or limited engagement preferred. Ability to collaborate in a high intensity ACT environment preferred.

Additional Requirements:

Current valid driver's license and good driving record required. Based on the Virginia DMV point system, records must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months. Out-of-state driving records must be obtained by applicant and presented at time of interview. Records must reflect at least three years of history and be dated within thirty days of interview date.

Pre-employment drug testing, FBI criminal background check and education/degree verification required.

Duties

Duties include but are not limited to the following:

  • Provides care coordination and assists individuals with serious mental illness to navigate benefits systems (SNAP, energy/cooling assistance, Social Security) and access community supports;
  • Links clients to community resources including primary care, housing supports, dental/vision care, transportation, financial programs, and other community services;
  • Tracks and monitors referrals and follow-ups;
  • Facilitates care coordination and follows up with external providers;
  • Works collaboratively with the multidisciplinary ACT Team to support client stability and recovery;
  • Actively participates in daily ACT team meetings to share updates, plan outreach, and coordinate services;
  • Documents all client contacts according to Medicaid, Department of Behavioral Health and Developmental Services (DBHDS), Community Service Board (CSB), and ACT Team requirements;
  • Completes annual required paperwork with clients; and
  • Performs other work as required.

This position is a part of an approved Career Development Plan (CDP) and offers career progression opportunities and salary incentives, as funding permits, based on performance, qualifications, and experience.
