Case Manager Assistant I | Part-time |Evening Shift | Venice

Job Summary:

Sarasota Memorial Health Care System is seeking a Case Manager Assistant I to join our Integrated Case Management team at our Venice campus.

This is an excellent opportunity for a healthcare professional who enjoys working with patients and families, coordinating services, solving problems, and supporting a multidisciplinary care team.

 

In this role, you’ll work alongside Case Managers to support:

• Patient and family communication
• Discharge and post-acute care coordination
• Medicare patient rights and required notifications
• Documentation and case management processes
• Communication with hospital teams and post-acute providers
• Patient throughput and transitions of care

 

What you’ll bring:

• Minimum of 2 years of customer service experience in healthcare or a related setting
• High school diploma, GED, or equivalent
• Strong communication and organizational skills
• Ability to collaborate effectively with patients, families, and healthcare professionals

Previous healthcare and discharge-planning experience is preferred.

 

You don’t need to be a nurse or licensed Case Manager to make an important impact on a patient’s transition of care.

 

If you have experience in Patient Access, healthcare customer service, medical office operations, care coordination support, insurance/authorization, hospital operations, skilled nursing, home health, or a related healthcare setting, your experience may translate well into this opportunity.

Required Qualifications:

- Require a minimum of two (2) years customer service experience in a healthcare or related setting.
- Require a High School Diploma, GED Equivalent, or Certificate; Associate's degree in related field preferred.

Preferred Qualifications:

- Prefer knowledge of computer systems.
- Prefer demonstrated ability in establishing and maintaining effective working relationships.
- Prefer demonstrated ability to accurately document in the medical records and various case management systems/software.
- Prefer demonstrated computer, keyboard and data input skills.
- Prefer demonstrated initiative and self-direction.
- Prefer demonstrated knowledge of Windows software preferred.
- Prefer effective oral and written communication skills.
- Prefer previous healthcare experience with discharge planning activities.

Work Days/Shift/Start Time:

This position is part-time, 20 hours per week on the evening shift.
