A social services organization in Brooklyn, New York is seeking a Case Manager to support residents through client-centered advocacy, care coordination, and service planning.
About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday (rotating Saturdays and one late day per week)
- Hours: 9am to 5pm (with one late day until 6/7pm per week; summer Fridays until 3pm when workload permits)
- Setting: Residential
Responsibilities:
- Manage a caseload of approximately 15 to 20 clients
- Conduct regular client meetings and assessments
- Connect clients with medical, mental health, and social service resources
- Support clients with appointments, groups, and daily living needs as needed
- Maintain accurate documentation, service plans, and assessments in an audit-ready manner
Qualifications:
- 2+ years of experience in Human Services and/or a related role
- Bachelor's Degree
- Bilingual (English and Spanish)
- Case Management background
Desired Skills:
- Experience working with Homeless populations
- Background supporting clients with Mental Health, Substance Use, or HIV/AIDS-related needs
- Familiarity with Entitlements and Public Benefits
- Experience with Crisis Intervention
- Proficiency with Documentation and Case Management systems
