Position Summary:
• We are seeking a compassionate and organized Case Manager to support adults residing in a residential housing program. 
• In this role, you will work directly with clients to help them achieve greater stability, independence, and overall well-being by coordinating services, advocating for their needs, and connecting them with community resources.
• The ideal candidate is comfortable working in a fast-paced residential environment, building trusting relationships, and responding calmly during crisis situations.

Responsibilities:
• Manage an assigned caseload of residential clients while providing ongoing case management and support.
• Meet with clients regularly to assess progress, identify needs, and develop individualized service plans.
• Coordinate referrals and connect clients with medical, behavioral health, housing, vocational, and community resources.
• Advocate for clients and assist with accessing benefits and other supportive services.
• Accompany clients to medical, psychiatric, or other essential appointments when needed.
• Maintain timely, accurate, and audit-ready documentation, including progress notes, assessments, and service plans.
• Facilitate or co-facilitate therapeutic, educational, or life skills groups and encourage resident participation.
• Respond appropriately to client crises by utilizing de-escalation techniques and coordinating emergency services when necessary.
• Conduct routine room and unit inspections while promoting a safe and supportive living environment.
• Assist with daily program operations, including meal service and client engagement activities, as needed.
• Maintain strict confidentiality and ensure compliance with agency policies and applicable regulations.
• Collaborate with multidisciplinary staff to provide high-quality, person-centered care.
• Perform additional responsibilities as assigned to support program operations.

Qualifications:
• Bachelor's degree required (Social Work, Human Services, Psychology, Sociology, or a related field preferred).
• Minimum of two years of experience working in human services, residential programs, behavioral health, housing, or a related setting.
• Experience supporting individuals experiencing homelessness, mental illness, substance use disorders, HIV/AIDS, or other complex social needs is highly preferred.
• Strong interpersonal, crisis intervention, and relationship-building skills.
• Excellent written and verbal communication abilities.
• Strong organizational skills with the ability to manage multiple priorities.
• Working knowledge of HIPAA and client confidentiality standards.
• Proficiency with Microsoft Office and electronic documentation systems.
• Flexibility to work occasional evenings, weekends, or holidays based on program needs.
• Bilingual Spanish required
