Case Manager
Job Description: Serving adults with traumatic brain injury and other physical and/or neurological impairments.
Desired Experience: Dependent on current Department of Health Qualifications
Experience in:
- case management
- writing service plans
- Linkages to outside resources
- Working with individuals with TBI or related injuries/disabilities
Education: Dependent on current Department of Health Qualifications
Minimum requirement is an Associates Degree + 2 years experience
Work Location:
THIS IS NOT A REMOTE POSITION. Participant's home or community (serving clients in Nassau and Suffolk counties) and Office work
Salary: Based on hours
Schedule:
- Day shift
- Monday through Friday
- Flexible
