A nonprofit services organization in Brooklyn, NY is seeking a Case Manager to provide client-centered support through advocacy, care coordination, and service planning. This role works with individuals experiencing homelessness and complex medical, mental health, and social service needs in a high-touch residential setting.
About the Opportunity:
Shift: Day shift
Schedule: Monday through Friday, with rotating Saturdays and one late day per week
Hours: 9am to 5pm (with one late day until 6:00 PM to 7:00 PM and summer Fridays ending at 3:00 PM, workload permitting)
Setting: On-site residential setting
Responsibilities:
Manage a caseload of approximately 15 to 20 clients
Conduct weekly client meetings, assessments, and service planning
Connect clients to medical, mental health, and social service resources
Escort clients to appointments and provide support with daily living needs as needed
Facilitate groups, track engagement, maintain documentation, and support crisis intervention and unit safety
Qualifications:
Bachelor's Degree
At least 2 years of human services experience
Experience with Case Management
Desired Skills:
Experience supporting homeless populations
Familiarity with mental health, substance use, HIV/AIDS, and public benefits
Strong communication, organization, documentation, and crisis intervention skills
