Short Description: The case manager provides crisis intervention, consultation, referrals, and brief interventions to patients. The role also completes follow-up with patients to assist in treatment linkages and navigating complex social service applications such as housing applications, disability enrollments, etc. Experience Required:

Previous acute care experience preferred.

Education Requirements:

Bachelor Degree -Social Work/Registered Nurse required. 

License/Certification Requirements:

• RN licensure in the State of New Jersey.
