Case Manager

Job Description:

The Case Manager is responsible for coordinating comprehensive care for patients receiving treatment for substance use disorders and co-occurring mental health disorders in residential and outpatient settings. The Case Manager ensures continuity of care, facilitates access to community resources, assists with discharge planning, and collaborates with the multidisciplinary treatment team to support patient recovery and successful reintegration into the community.The Case Manager works closely with therapists, medical staff, behavioral health technicians, and external providers to ensure treatment goals are achieved and barriers to care are addressed.

Responsibilities:

• Coordinate patient care across detox, residential, PHP, IOP, outpatient, and aftercare levels of care.
• Develop discharge and aftercare plans, coordinate referrals, and ensure follow-up appointments are scheduled before discharge.
• Connect patients with housing, employment services, medical care, transportation, insurance resources, and recovery supports.
• Collaborate with the treatment team, patients, families, and outside providers to address barriers to care and support treatment goals.
• Maintain accurate and timely documentation, including assessments, progress notes, care coordination notes, referral documentation, and aftercare plans.

Requirements/Qualifications:

• Bachelor’s degree in social work, psychology, sociology, behavioral health sciences, or a related field required.
• Minimum of 1 year of experience working in substance use disorder or mental health treatment.
• Experience using electronic medical records and Microsoft Office, including Word, Excel, and PowerPoint.
• Valid driver’s license in good standing and ability to support patient transportation needs.
• CPR and First Aid certification, or ability to obtain within 30 days of hire. SUD credential or higher state-specific license strongly preferred.
