Summary
The Case Manager for Community Supports (CS) & Enhanced Care Management (ECM) works as a member of the Youth for Change team and supports several programs within the agency. This position is a community-based, whole-person care role. As a professional case manager for CS and ECM this position will promote independence and well-being, conduct outreach, evaluate needs, and connect vulnerable Medi-Cal/ CalAIM populations with integrated physical, behavioral, social services, and making changes through skill development.

Experience and/or Education
  • Bachelor's Degree in Public Health, Human, Social Welfare or related field; or equivalent experience, training and coursework.
  • Minimum of one year of progressively responsible and direct work experience working with outreach and engagement, assessments, care planning, resource coordination, housing navigation, documentation and compliance among other experience.
  • Strong understanding of community resources and services
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Experience working with diverse populations including homeless individuals
  • Strong organizational and time-management skills
  • Possess a valid California Driver’s License with a clear driving record for the past three (3) years. 
The estimated base pay for Case Manager is $20.00 to $27.00; additional individual compensation may be available. Base pay is only a portion of the total rewards package, and a comprehensive benefit program is available for qualifying position.
