Summary The Case Manager for Community Supports (CS) & Enhanced Care Management (ECM) works as a member of the Youth for Change team and supports several programs within the agency. This position is a community-based, whole-person care role. As a professional case manager for CS and ECM this position will promote independence and well-being, conduct outreach, evaluate needs, and connect vulnerable Medi-Cal/ CalAIM populations with integrated physical, behavioral, social services, and making changes through skill development. The estimated base pay for Case Manager is $20.00 to $27.00; additional individual compensation may be available.
