About the Role: The Case Manager is an integral member of Community Reach Center's Outpatient, Community programs, and Case Management Teams. The Care Manager role will emphasize assisting persons to explore and develop social, occupational, leisure, and living skills. Provides services that supports consumers in living in the community successfully including linking consumers with resources to obtain and/or learn basic needs.Essential Functions: Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and familiesEffective and timely communication across multi-disciplinary teams including external partnersAccountability to expectations held by external partners not just those of Community Reach CenterProvides outpatient restoration education services for consumers going through competency court proceedings.Support and closely coordinate mental health and substance abuse care with the consumer's clinical care team and the primary care providerAttend collaborative treatment team meetings and reviews of consumersProvides crisis de-escalation when neededAssist eligible consumers in gaining access to needed medical, social, educational, and other servicesProvide an assessment of needs and service plan developmentEmpowers consumers to reach their goals and sets a positive example and provides role modeling as well as technical expertise to accomplish goalsProvide transportation for consumers in agency vehicles when neededWill work closely with outside organizations on resources and care coordinationUsing an evidence-based model of measurement-based care tracks success for each individual consumerQualifications:Requires bachelor's degree Human Services or related fieldPrevious case management experience strongly preferredPrevious Experience in court and legal proceedings preferredCommunication, organization, time management and clinical skills.Ability to work independently in a community-based setting.Valid Colorado Driver's license with a clean driving record.Bilingual Spanish preferred, but not requiredPhysical Requirements:Regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and keyboard.Specific vision abilities required by this job include close vision requirements due to computer work.Light to moderate lifting is required,Regular, predictable attendance is required.Schedule:Monday- Thursday 8am-6pm, Friday 8am-6pmSalary Information:$24.04 to $27.00/jhrEligible for $250/month Bilingual StipendApplication Deadline:Accepting Application on an on-going basis
