Case Manager

Hours: 30-40 hours per week, Monday - Friday 8:00 - 5:00

CHA is hiring a highly motivated, creative, and flexible individual to provide client support and case management across various locations including clinical, community, detention and harm reduction settings.

Primary Responsibilities:

• Provide case management
• Conduct intakes and enrollment activities
• Follow care plans for clients
• Complete data tracking and monthly reports
• Document appropriately in electronic health record
• Accompany clients to meetings and participate in meetings of community groups to advocate client needs

Education/Experience:

• Bachelor's degree in a human service field or similar degree with two years of experience with incarceration, probation, substance use recovery, mental health treatment or homelessness OR
• High school diploma or equivalent with 4 years of experience with incarceration, probation, substance use recovery, mental health treatment or homelessness and a peer support specialist certification OR equivalent education or experience.
• Lived experience preferred.

Knowledge, Skills & Abilities:

• Knowledge of harm reduction principles, recovery and person first language
• Ability to plan and work collaboratively with community partners and CHA staff in person and virtually
• Ability to work with diverse groups of people and to deal tactfully with others
• Ability to exercise good judgement in appraising situations and making decisions
• Accurately record services rendered and to interpret and explain records
