Join our mission-driven team as a Full-time Case Manager at our Santa Barbara Jail Program. This individual will be responsible for general case management duties including intake, assessment, case record management, and discharge preparation.
What You'll Do:
- Within five (5) days of referral, collect all intake information and enter into agency data collection systems on the day the data is obtained.
- Perform client needs assessments within five (5) days of intake and enter data into CSI data systems on the day the data is obtained.
- Make referral to groups (as appropriate) within fifteen (15) days from the date of intake and orientation.
- Actively participate in Case Planning Team Meetings by presenting new clients, assessments, and Individual Service Plan updates on clients as well as discharge planning for clients nearing discharge. Provide and elicit peer support for client issues.
- Develop and document program plan goals with clients within five (5) days of intake and review with client on a bi-weekly basis. Monitor treatment stipulations, job readiness preparation, and community resource referrals.
- Schedule, conduct, and document individual case management sessions that address client personal issues and program performance. Enter each session into CSI data systems on the day the data is obtained.
- Based on workflow of the office, a CM may be assigned group facilitation responsibilities.
- Responsible for entering all new referrals to interventions they facilitate into the CSI data systems on the day of the referral.
- Prepare, plan, and facilitate individual and group sessions with integrity and fidelity to curriculum/manual.
- Schedule and conduct make-up sessions as needed.
- Documentation of these group and individual sessions are to be entered into the agency data systems on the same day the information is received.
- Must maintain a minimum of 75% completion rate for each intervention facilitated.
- Maintain client files and ensure compliance with agency policy regarding confidential information. This confidentiality must be maintained even if you are no longer an employee of CSI.
- Submit monthly progress reports for clients to your supervisor four (4) days before due date.
- Complete incident reports as needed and submit to supervisor prior to leaving shift.
- Tape all sessions with clients and follow procedure to upload these recordings for quality assurance review.
- Actively participate in quality assurance review session and apply new strategies and techniques presented.
- Register for and attend all required trainings and boosters including but not limited to Motivational Interviewing, Case Management, and Assessment.
- Provide coverage of front desk and perform the associated duties.
- Participate in the administration and collection of WAI surveys.
- Secure urines from same sex clients and complete all required paperwork and documentation including agency data collection systems on the day the data is obtained.
- Monitor case load to ensure all urines and breathalyzers are requested and are random.
- Serve as a role model to clients and staff modeling pro-social behaviors.
- Maintain a working knowledge of and abide by the Agency's programs, policies, procedures, and code of ethics.
- Perform other tasks as assigned by the supervisor.
Schedule and Salary:
- Schedule: Tuesday-Friday 6:00am to 4:00pm
- Salary: $30.00 per hour

Qualifications:
- Bachelor's degree and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.
- Excellent oral, written, and interpersonal skills.
- Ability to engage and develop collaborations with community agencies.
- Previous assessment experience preferred.
- Experience with Microsoft Office.
