    What We're Looking For

    Detroit Recovery Project is seeking a Case Manager to facilitate prevention education services and provide individualized case management support to program participants. Under the supervision of the Prevention Project Coordinator, this position screens individuals for case management services, assists with identifying and recruiting participants for prevention education programming, and connects individuals with appropriate community resources and services.

    The Case Manager works directly with participants to identify strengths, needs, and goals; develop individualized plans; provide referrals and linkages to care; and support participants as they work toward greater independence and well-being.

    Compensation Range: This position is supported through grant funding. As a result, compensation is determined by the grant budget and may fall within a designated pay rate or range set by that funding source. The pay range for this position is $24.04-$25.24/hour and is based on non-discriminatory factors such as skills and experience. This position is hourly non-exempt and is eligible for overtime compensation with prior approval from management.

    The Ideal Candidate Will Perform The Following:

    Case Management & Participant Support

    • Assist and empower participants in identifying their primary concerns and developing strength-based independent living plans with measurable goals and objectives.
    • Develop individualized client treatment plans and support participants in working toward identified goals.
    • Maintain a minimum caseload of ten (10) clients.
    • Screen participants to determine eligibility and need for case management services.
    • Provide referrals and link participants to appropriate community-based resources and services.
    • Provide warm handoffs whenever possible to promote continuity of care and successful service connections.

    Community Outreach & Prevention

    • Conduct direct outreach within the community to provide prevention services and recruit program participants.
    • Facilitate prevention education series and other program activities.
    • Provide HIV and HCV testing and connect individuals to appropriate care and resources.
    • Assist with identifying and recruiting participants for prevention education programming.
    • Coordinate prevention services with outside organizations, community partners, and groups.

    Program Documentation & Data

    • Administer required pre- and post-program evaluation tools.
    • Enter participant and program data into DRP systems accurately and in a timely manner.
    • Maintain appropriate documentation related to participant services, referrals, goals, and program activities.
    • Support program reporting and performance requirements as assigned.

    Collaboration & Professionalism

    • Maintain positive and professional interpersonal relationships with staff, peers, participants, volunteers, and community partners.
    • Demonstrate teamwork, initiative, and a willingness to learn.
    • Provide strong customer service consistent with DRP's mission, values, and organizational expectations.
    • Demonstrate and actively promote an understanding of and commitment to the mission and values of Detroit Recovery Project.
    • Perform other related duties as assigned.

    Qualifications

    Required

    • Bachelor's degree in Human Services or a related field.
    • Minimum of one (1) year of professional case management experience, including knowledge of services and resources available within the communities DRP serves.
    • Certified HIV Tester in the State of Michigan.
    • Valid and unrestricted Michigan Driver's License.
    • Strong organizational and time-management skills.
    • Strong interpersonal and communication skills.
    • Ability to work collaboratively as part of a multidisciplinary team.
    • Demonstrated initiative and willingness to learn.
    • Strong customer service skills consistent with DRP's core values.
    • Proficiency with Microsoft Office Suite, including Word, Excel, PowerPoint, and Access.

    Preferred

    • Qualified Mental Health Professional (QMHP).
    • Training in Motivational Interviewing.
    • Experience working in prevention, behavioral health, substance use disorder, recovery support, or community-based services.
