Position Description

Under general supervision, the Case Manager I provides case management and peer support services to Veterans who find themselves justice involved within Harris County. The role supports mental and physical health, including conducting basic assessments, exit planning and connection in facilitating service referrals, and maintaining documentation. Individuals in this role are responsible for managing a caseload of clients with routine needs, coordinating services with providers, and monitoring progress under established guidelines.

This position requires foundational knowledge of case management and community resources, strong organizational and communication skills, and the ability to follow procedures and work collaboratively to perform the essential functions of the position.

Duties and Responsibilities:

  • Client Assessment and Intake: Conducts initial screenings and basic assessments to identify client needs, eligibility, and risk factors using standardized tools.

  • Service Planning and Referral: Assists in developing service plans, provides information on available resources, and makes referrals to appropriate programs and providers.

  • Care Coordination: Coordinates routine services for clients, including scheduling appointments, confirming participation, and communicating with providers and support systems.

  • Monitoring and Follow-Up: Monitors client progress toward goals through regular contact, documents changes, and updates service plans in consultation with supervisors.

  • Documentation and Recordkeeping: Maintains accurate and timely case notes, forms, and records in accordance with program standards and confidentiality requirements.

  • Client Education and Support: Provides basic education on program requirements, health or behavioral health topics, and self-advocacy strategies to clients and families.

  • Problem-Solving and Resource Navigation: Assists clients in overcoming routine barriers to care about by identifying solutions and linking them to additional supports.

  • Team Collaboration: Participates in case conferences, supervision, and team meetings, shares information, and coordinates with internal and external partners to support client outcomes.

Requirements

Education:

  • Bachelors degree in Social Work, Public Health, Psychology, Human Services, or a closely related field; or an equivalent combination of education and experience.

Experience:

  • Military Veteran, experience working with Veterans and dependents, military core cultural understanding.

Knowledge, Skills, and Abilities:

  • Knowledge of basic case management principles, practices, and documentation standards.

  • Knowledge of community resources and service systems relevant to public and behavioral health.

  • Knowledge of confidentiality and privacy requirements related to client information.

  • Skilled in establishing rapport, interviewing clients, and gathering information in a respectful and culturally responsive manner.

  • Skilled in organizing work, managing time, and maintaining detailed records for multiple clients.

  • Ability to follow policies, procedures, and supervision while exercising initiative in routine problem solving.

  • Ability to communicate clearly with clients, families, staff, and service providers both verbally and in writing.

  • Ability to recognize signs of risk or crisis and promptly seek guidance or refer to appropriate resources.

  • Ability to work effectively as part of a multidisciplinary team.

  • Ability to use computers and standard software applications for documentation, scheduling, and communication.
