This person provides case management services to individuals of all ages with a primary diagnosis of intellectual or developmental disability. This includes assessing service needs; developing plans for access to services; liaison with client, families, programs, and service providers; ongoing monitoring or client service needs; advocacy; and consultation and education to clients, families and the community. **4,500 Sign on Bonus**Starting Hourly Rate:Case Manager I -$16.73Case Manager II-$17.56Case Manager III-$18.44Education/Experience Required:- High school diploma with at least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or intellectual disabilities is required.
- Bachelor's degree in psychology, counseling, social work, special education, or other Virginia DMAS approved related human services field is preferred.
- Experience providing professional supports to individuals with intellectual disabilities is preferred.
