We meet clients where they are, supporting housing search, move-in costs, and ongoing stabilization so clients can build self-sufficiency and long-term success.
The Role
As a Case Manager I, you will provide hands-on support to individuals, families, and transitional-aged youth (TAY) as they move into stable housing. You will work closely with clients to create personalized plans, connect them to essential resources, and provide guidance and advocacy that empowers long-term success. This is a role for someone who is compassionate, client-centered, and energized by helping people achieve meaningful change.
