Case Manager II – Probation

Location: Hanford, CA
Position Type: Full-Time, Non-Benefited
Salary Range: $25.34 – $26.88 per hour

Position Overview

Provide services to probation-involved youth and their families through case management, advocacy, rehabilitative services, care coordination, and linkage to community resources. This position works closely with clinicians, schools, probation, and community partners to support positive outcomes.

Key Responsibilities

  • Advocate for and assist probation-involved youth and families in accessing behavioral health, medical, educational, housing, and community services.
  • Work closely with Mental Health Clinicians to implement individualized care plans.
  • Provide guidance, mentoring, and support to consumers in achieving goals.
  • Develop and facilitate rehabilitative skills groups.
  • Participate in Child and Family Team (CFT) meetings and collaborative care planning.
  • Conduct home, school, and community-based visits as needed.
  • Assist consumers with Medi-Cal eligibility and benefit continuation.
  • Maintain documentation within required timeframes and productivity standards.
  • Build positive relationships with community agencies and service providers.
  • Utilize electronic health records and required software systems effectively.

Required Qualifications

  • Associate Degree plus four (4) years of experience; OR Bachelor Degree plus two (2) years of experience; OR LPT/LVN/RN plus four (4) years of experience.
  • Valid California Driver License (Class C).
  • Strong communication, interpersonal, organizational, and documentation skills.
  • Ability to work independently and collaboratively with multidisciplinary teams.

Preferred Qualifications

  • Bilingual English/Spanish preferred.
