The Case Manager II for Adult Mental Health Services, provides case management and clinical services for individuals with mental illness, substance disorders use and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with: individuals served, families, additional service providers, and other community agencies as needed to ensure quality services. Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.
Case Manager II-Bachelor's Degree RequiredStarting Hourly Rate: $17.56--Depending on Experience
Case Manager III-Bachelor's Degree and QMHP RequiredStarting Hourly Rate: $18.44--Depending on Experience
Education/Experience Required:- Bachelor's Degree is required in psychology, counseling, social work or special education, but will also consider an unrelated degree.
- Experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is preferred
Additional compensation will be considered for any new employees who speak multiple languages.
