Position Overview
Under the direct supervision of the primary care provider, the Case Manager II (Registered Nurse) is responsible for providing medical case management services to a specified panel of patients within the clinical setting.  This position will also perform Medicare Annual Wellness Visits (AWV).  A portion of time will be spent providing case management services for patients utilizing the Housing Opportunities for People living with HIV/AIDS (HOPWA) grant.     
 
THIS POSITION OFFERS A 40-HOUR WORKWEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-ANNUAL BONUS, BI-WEEKLY PAY.

Essential Tasks
  • Provide medical case management and non-medical case management services to people living with HIV.
  • Perform annual comprehensive assessments to evaluate patients health care needs and social determinates of health in order to develop individualized and person-centered care plans using a collaborative approach including patient and family participation, where possible.
  • Update and document each care plan as the patients’ needs change and/or to address gaps in care; including, at a minimum, review and revision upon re-assessment.
  • Identify patients with recent emergency department visits or hospital admissions, and assists them with transitioning back to their primary care provider, including providing medication reconciliation and assisting with medication access.
  • Provide reviews and reinforce patient education regarding preventative care, chronic conditions, medications, dietary restrictions and other therapeutic regimens.
  • Work toward improving overall quality metrics rating by bringing members into compliance and closing gaps in care.
 
Experience and Education
  • Graduate of an accredited school of nursing as a Registered Nurse.
  • Current and unrestricted license to practice in North Carolina.
  • Previous experience, preferably, in a community health center setting in the area of nursing and/or case management.
  • Effective communication and interpersonal skills.
  • Must demonstrate the skills and judgement necessary to provide direct care to patients and nursing interventions under the direct supervision of providers.
  • Strong computer skills and working knowledge of windows-based environment and Electronic Health Record (EHR) experience.

Physical Requirements
  • Constant standing and walking, occasional bending, stooping, lifting, reaching forward and overhead.
  • May occasionally be required to lift patients, office equipment and paper, usually not exceeding fifty pounds of direct lift.
  • Frequent computer work required.

Base Pay Overview
The starting pay for this position is $31.50 hourly. Please keep in mind that actual salary will vary based on various factors such as qualifications, skills, competencies, and proficiency for the role.
