We are seeking a compassionate, resourceful, and motivated Case Manager - Intensive Housing to support individuals and households experiencing unsheltered
homelessness and other forms of homelessness that meet Category 1 eligibility
criteria. In this rewarding role, you will help people secure housing, maintain
successful tenancies, connect with essential resources, and move toward greater
independence.
As a Case Manager, you will build trusting
relationships, advocate for the people you serve, collaborate with landlords
and community partners, and provide individualized support when it matters
most. From helping someone find an apartment to preventing an eviction or
navigating a crisis, your work will have a direct impact on housing stability
and quality of life.

What You'll Do
Support Housing Stability
- Help
individuals and households maintain safe, stable housing and develop the
skills and resources needed for long-term tenancy.
- Assist
people in locating available apartments in the private rental market when
housing has not yet been secured.
- Provide
intensive support during the initial months of tenancy and whenever
additional assistance is needed.
- Work
with landlords and property management teams to address concerns, resolve
conflicts, and prevent evictions.
- Help
individuals complete lease renewals, housing recertifications, income
verifications, and other required documentation.
Provide Person-Centered Case Management
- Assess
individual needs related to mental health, substance use, physical health,
personal care, housing maintenance, and other areas affecting housing
stability.
- Develop
and update an Individualized Service Plan with each person served every
six months.
- Complete
at least four community-based contacts per person served each month,
including a minimum of two home visits.
- Apply
recovery-oriented, person-centered, and harm reduction approaches to
support individual goals and promote independence.
- Recognize
when additional services or higher levels of care may be needed and
coordinate appropriate referrals.
Connect People With Resources
- Link
individuals to community resources, including mental health services,
substance use treatment, energy assistance, eviction prevention, moving
assistance, employment support, job training, and volunteer opportunities.
- Participate
in Housing Services navigation workgroups to share resources, troubleshoot
challenges, and coordinate support.
- Encourage
participation in resident advisory groups, community committees, and
advocacy opportunities related to housing, mental health, disability
rights, substance use services, and social justice.
Coordinate, Advocate, and Document
- Maintain
consistent communication with landlords, property managers, service
providers, and community partners.
- Fulfill
designated housing-related proxy responsibilities when authorized and
required, including participation in initial eligibility interviews and
annual recertifications.
- Maintain
accurate, timely, and thorough case notes, service plans, electronic
health records, and other required documentation.
- Follow
agency policies, professional standards, funding requirements, and
applicable confidentiality expectations.
What We're Looking For
We are looking for someone who combines empathy with
practical problem-solving skills and understands the importance of meeting
people where they are.
Required Qualifications
- A
bachelor's degree is preferred.
- In
lieu of a bachelor's degree, an associate degree and at least one year of
experience working with homeless populations are required.
- Exceptions
to education and experience requirements may be considered on a
case-by-case basis with prior approval from Housing Services.
- A
valid driver's license, access to a personal vehicle, and sufficient
automobile insurance to meet agency requirements.
- Ability
to meet all applicable agency and contractor requirements for criminal and
caregiver background checks, driving record review, and drug screening.
Preferred Experience
- At
least one year of experience working with individuals experiencing mental
health challenges, substance use concerns, and/or co-occurring needs.
- Experience
in case management, housing services, homelessness prevention, supportive
housing, behavioral health, or community-based social services.

health records, and case documentation systems.

confidentiality, agency values, and respectful collaboration.
Work Environment and Physical Requirements
This is a mobile, community-based position serving
individuals throughout Milwaukee County. Work takes place in an office,
individuals' homes, and other community settings.
- Full-time
schedule averaging 40 hours per week.
- Regular
local travel for home visits, community appointments, meetings, and other
service-related activities.
- Transportation
of individuals served may be required.
- A
flexible schedule is necessary to respond to service needs.
- Ability
to navigate stairs and move throughout community settings.
