General DescriptionResponsible for providing supportive counseling and providing training in independent living skills to residents in the Apartment Treatment Program. Duties and ResponsibilitiesTeach residents independent living, tenancy, housekeeping, socialization, vocational, educational, and community-integration skills that support independence and psychiatric stability.Coordinate resident admissions, transfers, and discharges, and provide weekly face-to-face field services and regular apartment meetings.Partner with residents to develop, evaluate, and update person-centered service plans and strength-based individualized goals.Provide supportive counseling, crisis intervention, case management, collateral contacts, and referrals or linkages to clinical, vocational, educational, recreational, and community resources.Support medication self-administration, teach medication effects and emergency procedures, and promptly notify supervisors of medical, psychiatric, or programmatic emergencies.Promote safe, well-maintained housing by inspecting apartments, coordinating approved repairs, monitoring fire-safety equipment, and teaching fire safety and disaster preparedness.Provide transportation and distribute resident funds as authorized; coordinate treatment-team activities; complete documentation promptly; attend required meetings and training; and follow agency policies and other assigned duties.
