Avalon Wellness & Recovery Center is seeking a compassionate, organized, and recovery-oriented Case Manager to join our team. The Case Manager plays a critical role in helping individuals navigate the recovery process by connecting them with services, resources, and support systems that promote long-term success.
The ideal candidate will have experience working with individuals affected by substance use disorders and co-occurring mental health conditions, possess strong advocacy skills, and demonstrate a commitment to person-centered, trauma-informed care.
Conduct assessments to identify client needs, strengths, barriers, and recovery goals.
Develop individualized case management plans that support recovery, stability, and community integration.
Coordinate care with counselors, medical providers, peer support professionals, family members, and community partners.
Assist clients in accessing housing, transportation, employment, healthcare, benefits, legal services, and other community resources.
Provide ongoing support, encouragement, and advocacy throughout the recovery process.
Monitor client progress and adjust service plans as needed to support successful outcomes.
Maintain accurate, timely, and professional documentation in the electronic health record (EHR).
Participate in treatment team meetings and contribute to interdisciplinary care planning.
Facilitate educational groups and resource-focused sessions as assigned.
Support discharge planning and continuity of care.
Maintain knowledge of community resources, recovery support services, and best practices in substance use disorder treatment.
Uphold all confidentiality requirements, including HIPAA and 42 CFR Part 2 regulations.
Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Criminal Justice, or a related field.
Master's degree preferred.
Minimum of four (4) years of experience working in behavioral health, substance use disorder treatment, recovery support services, or case management.
Knowledge of substance use disorders, co-occurring disorders, trauma-informed care, harm reduction principles, and recovery-oriented systems of care.
Strong communication, organizational, and documentation skills.
Ability to establish professional relationships with clients from diverse backgrounds.
Experience utilizing electronic health record systems.
Valid driver's license and reliable transportation.
CADC, CRADC, CAC, CPRS, CRPR, QMHP, or similar credential.
Experience working within ASAM levels of care.
