Summary:
The Case Manager, regardless of type, is responsible for the support and mentoring of clients while in transition from IOP, or Outpatient Treatment through discharge. The purpose of the Case Manager role is one of instruction, care coordination, support, and meeting the client's health care needs while promoting quality effective outcomes. Each client is provided with an individualized plan that serves as an integral phase in the Companies continuum of care. It is the role of the Case Manager to support the client as they navigate all phases of treatment and reintegrate back into the community as sober and healthy individuals. The case manager should ensure this transition is safe and appropriate.

Position Type and Expected Hours of Work
This is a full-time hourly position in a residential treatment facility setting that is open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather will be required.
Minimum Qualifications:
Case Manager will be provided performance evaluations to measure performance areas. Inability to meet performance expectations will result in specific, short term performance enhancement action plan with a timeline to improve. Below average rating in employee performance will result in progressive disciplinary action up to and including termination of employment.
Pay: $23.00 per hour
