Case Manager LPN

Pride Health is seeking a Case Manager LPN for one of our clients in San Diego, CA! (Contract)

Schedule: Mon-Fri 8:30am-5pm

Contract: 6-month contract

Pay Rate: $30-32 per hour

Duties and Responsibilities

• Care coordination
• Track and respond to all incoming referrals
• Screening of all incoming clients with a medical need
• Schedule medical appointment and transportation

Requirements and Qualifications

• LPN license CA
• 1+ years of Case Management/Care Coordination experience
