The Case Manager LVN at Mission Community Hospital plays a critical role in coordinating and managing patient care to ensure optimal health outcomes. This position involves comprehensive assessment, planning, implementation, and evaluation of patient care plans in collaboration with multidisciplinary healthcare teams. The Case Manager LVN acts as a liaison between patients, families, and healthcare providers to facilitate effective communication and continuity of care. By monitoring patient progress and advocating for necessary resources, this role helps reduce hospital readmissions and supports efficient utilization of healthcare services. Ultimately, the Case Manager LVN contributes to enhancing patient satisfaction and quality of care within the hospital setting.

Minimum Qualifications:

  • Valid Licensed Vocational Nurse (LVN) license in the state of California.
  • Minimum of 2 years of clinical nursing experience, preferably in a hospital or healthcare setting.
  • Basic knowledge of case management principles and patient care coordination.
  • Strong understanding of medical terminology and healthcare documentation standards.
  • Current CPR certification.

Preferred Qualifications:

  • Experience in case management or discharge planning within an acute care environment.
  • Familiarity with electronic health record (EHR) systems and healthcare information technology.
  • Certification in case management (e.g., CCM) or related specialty.
  • Excellent interpersonal and communication skills to effectively interact with diverse patient populations.
  • Ability to work collaboratively within multidisciplinary teams.

Responsibilities:

  • Conduct thorough patient assessments to identify healthcare needs and develop individualized care plans.
  • Coordinate with physicians, nurses, social workers, and other healthcare professionals to implement and adjust care plans as needed.
  • Monitor patient progress and document clinical information accurately in medical records.
  • Provide education and support to patients and their families regarding treatment plans, medications, and post-discharge care.
  • Facilitate communication between patients, families, and healthcare teams to ensure seamless care transitions.
  • Assist in discharge planning by arranging necessary services and follow-up appointments.
  • Advocate for patient needs and coordinate access to community resources and support services.
  • Ensure compliance with hospital policies, regulatory requirements, and quality standards.
