Case Manager - Lynwood

A Case Manager provides case management and behavioral guidance for program youth.Persons employed as case managers are responsible for behavioral case management andlinkage services for the youth and families being served as well as skill building for individualrehab services. The position involves working designated shifts under the direct supervision ofthe Clinical Supervisor or Program Director.DUTIES:1. CASE MANAGEMENT: Provides general case management and skill building for client'sand their and families in accordance with the policies of the program andreferring/regulatory agencies (DMH, DCFS, Probation, schools).2. OBJECTIVES: Develops measurable case management and individual rehab objectivesfor clients as outlined in the Client Care Plan and monitors progress toward those goals.3. PRODUCTIVITY: Completes thorough and timely documentation of all contacts withclients and maintains productivity standards set forth by the agency Tier System andinternal policies.4. MEETINGS: Attends all staff meetings, in-service training and conferences as requiredand scheduled.5. CONSULTATION: Regularly consults with minor's parent or guardian, probation officeror CSW, and school staff.6. TREATMENT TEAM: Meets regularly with other members of the treatment teamconcerning client and program issues and coordinates needed services and linkages.7. CONFIDENTIALITY: Maintains strict confidentiality over all records and informationrelative to minors.8. RESPECT: Demonstrates respect for the intrinsic value of each person, their individualdifferences and culture.9. CRISIS: Ensures the safety of youth. Provides direct crisis intervention and therapeuticguidance.10. CLIENT INTEREST: Stimulates client's interests in various clinical offerings. Monitorsprogress and maintains close contact with client and family.11. BOUNDARIES: Establishes positive, respectful and nurturing relationships with clientsand families within professional boundaries and in accordance with agency policies andprocedures.12. TRANSLATION: Provides translation services as needed.13. STRENGTH BASED: Is capable of accurately identifying and building on the strengths ofclients to better reach their potential.14. TRAVEL: Travels independently to various environments (buildings with stairs, variouscommunities, schools, etc.) to provide services as needed.15. OTHER: Performs other duties as assigned by supervisor. QUALIFICATIONS:1. Bachelor's degree related to the fields of psychology, social work or other majors definedby the Department of Mental Health as a related degree.2. Valid California Drivers License, good driving record, and must be able to drive to fulfillthe duties of the position.
