Job Description
Summary of Responsibilities
The Case Manager is a direct service role involving the application of casework principles and frequent client interaction. As part of the treatment team, the Case Manager is responsible for coordinating services for individuals with mental illness. This includes training and educating clients and their families about symptoms, treatments, and available resources, as well as providing support during crisis situations. The level and frequency of contact are tailored to the individual’s specific needs and level of functioning.
Key Responsibilities
Service Delivery:
- Participate in clinical staff meetings to support treatment planning and coordination.
- Conduct assessments to identify client strengths, needs, and eligibility for case management services.
- Develop individualized case plans that outline client goals and the steps necessary to achieve them.
- Deliver case management services as specified in the case plan, ensuring alignment with client goals.
- Arrange transportation for clients to facilitate access to services and achieve case plan objectives.
Support and Education:
- Make referrals and schedule client appointments with appropriate service providers.
- Offer ongoing supportive services to clients, including advocacy and emotional support.
- Provide training in Basic Living Skills, Family Support and Education, and Crisis Intervention/Resolution services as outlined in the clinical treatment plan.
Collaboration and Documentation:
- Collaborate with therapists and other service providers to identify and engage clients in need of services.
- Maintain accurate, current documentation of all services provided.
- Submit billing data promptly to ensure accountability and compliance with organizational procedures.
Additional Responsibilities:
- Drive to client homes and community locations as required.
- Navigate stairs or other physical environments in client homes or service sites.
- May assume supervisory or leadership responsibilities as assigned by management.
Supervisor
The Case Manager reports to a Program Coordinator or Program Director.
Required Qualifications
- Bachelor’s degree in a human services field.
- Completion of an approved training program.
- Valid driver’s license and reliable transportation.
