The Center for Urban Community Services (CUCS) is a leading NYC nonprofit dedicated to ending homelessness through integrated housing, health, and social services. Our team empowers New Yorkers with complex needs to achieve stability, independence, and well-being through compassionate care.

Position Summary

CUCS invites applications for the Case Manager position at The Park House, located in the Tremont neighborhood of the Bronx. Supporting 20 residential units for formerly homeless individuals and special needs populations (including individuals living with HIV/AIDS), the Case Manager delivers person-centered care to help clients maintain permanent housing and achieve recovery goals.

Key Responsibilities

  • Direct Care & Planning: Maintain collaborative client relationships; create and update Individualized Living Plans (ILPs); assess client needs and support daily living skills, housekeeping, pest mitigation, and medication self-administration.

  • Benefits & Housing Preservation: Assist clients with securing public benefits (Medicaid, SNAP, SSI/SSDI); support permanent housing retention, annual recertifications, apartment inspections, and tenancy maintenance.

  • Community Outreach & Crisis Support: Deliver field/onsite services; accompany clients to medical, legal, and benefits appointments; provide crisis intervention and coordinate with hospitals and emergency providers.

  • Programming & Documentation: Facilitate group activities and evidence-based interventions; complete timely progress notes, ILPs, and benefit records per funder and agency regulations.

Qualifications

  • High School Diploma with relevant experience, or Bachelor's degree (BSW/BS/BA).

  • Experience with supportive housing, HIV/AIDS populations, mental health, or substance use preferred.

  • Crisis intervention, communication, and organizational skills.

  • Computer literacy required.

Responsibilities

  • Client Engagement & Service Delivery: Develop service plans (ILPs), provide regular outreach, support medication self-administration, and escort clients to medical, legal, and benefit appointments.

  • Benefits & Housing Retention: Help clients secure public benefits (SSI/SSDI, Medicaid, SNAP), manage rep payee funds, handle recertifications/inspections, and assist with daily living skills (ADLs, cleaning, pest remediation).

  • Crisis Response & Operations: Deliver trauma-informed de-escalation, coordinate emergency care, run support/recreational groups, and maintain timely EHR progress notes for client contact targets.
