    Job Description


    The Case Manager plays a crucial role in providing compassionate support and guidance to youth in care. This person

    is the primary case manager for the youth assigned to them and is responsible for ensuring that all services specified

    in the youth's case plan are delivered as required. This individual is expected to take an active role in the development

    and fostering of relationships between youth and staff. This person is expected to display and maintain a high

    standard of ethical behavior and professionalism and function as a member of a team in providing a balanced

    approach to planning and assessment for VisionQuest youth.



    ESSENTIAL FUNCTIONS:

    Case Management Operations:

    o Oversee the comprehensive management of case services, ensuring strict adherence to

    protocols for youth evaluations and assessments

    o Maintain meticulous documentation to guarantee compliance with VisionQuest, State, and

    Federal regulations

    o Conduct thorough reviews of all assessments and related documents for precision and

    correctness prior to submission

    o Ensure the timely and accurate submission of all reports and deliverables in accordance

    with state licensing requirements and VisionQuest's policies and procedures

    Communication and Coordination:

    o Maintain regular contact with various stakeholders involved in the youth's care

    o Coordinate with service providers and community resources to ensure comprehensive care

    o Assist with referrals and coordination of any specialty services

    o Maintain professional communication with all stakeholders, including family members

    and guardians

    Service Delivery:

    o Monitor and review youth Program Plans and progress

    o Understand the individual strengths and needs of each youth on caseload

    o Provide supervision and support for youth activities as required

    o Maintain security and confidentiality of all youth information including records

    Compliance:

    o Document all case management actions and communications as required

    o Maintain an in-depth understanding of VisionQuest and licensing policies and procedures

    o Monitor timeliness and maintain content of all reports, letters, and notifications

    o Other tasks and responsibilities as assigned by supervisors

    SKILLS & ABILITIES:

    Excellent interpersonal communication skills with ability to listen actively

    Commitment to professional development and proactive research

    Ability to work independently and as part of a team

    Strong computer skills (Microsoft Office, Google Workspace, Google Drive) with the ability to learn

    new software

    Excellent writing skills including the ability to produce comprehensive narrative reports



    EDUCATION & EXPERIENCE:

    Bachelor’s degree in the behavioral sciences, human services or social services fields strongly preferred.

    Child welfare and/or case management experience is strongly encouraged.

    A minimum of one year of related work experience, preferred.



    QUALIFICATIONS:

    Must be 21 years of age or older.

    Flexible schedule (evening, weekends, holidays) to facilitate program services.

    Cleared tuberculosis test results.

    May be required to pass a drug test.

    Cleared background checks from appropriate entities, including Child Abuse and Neglect and FBI

    criminal background.

    Obtain and maintain CPR/First Aid and Crisis Prevention Intervention (CPI) certification.

    Must have a valid driver’s license and be eligible to drive to facilitate program services as required.



    WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

    May lift and carry up to 10 pounds frequently, up to 20 pounds occasionally, up to 50 pounds

    infrequently.

    Performs tasks that may involve standing or walking 50% of the day; sitting 50%of the day.

    May bend, stoop, reach and grasp as necessary.

    Must be able to provide First Aid and CPR.

    Must be able to perform physical holds as instructed in Crisis Prevention Intervention training

    (CPI).
