    First Responder Wellness is dedicated to supporting the behavioral health and overall well-being of first responders and their families through compassionate, confidential, and evidence-informed care. We are seeking a Case Manager who is passionate about recovery and understands the unique challenges of high-stress and high-trauma professions.

    Schedule: Tuesday through Saturday

    Position Overview

    The Case Manager provides comprehensive support throughout participants' treatment and recovery journey, including assessments, treatment planning, counseling, and coordination of care. This role builds trusting relationships, promotes active engagement in treatment, and helps participants develop the skills and resources needed for long-term recovery. The Case Manager collaborates with families, outside providers, and the clinical team to ensure continuity of care and successful outcomes

    Key Responsibilities

    • Orient new participants to the program, including expectations, available services, treatment goals, and the resources available through First Responder Wellness.
    • Build supportive, professional relationships with participants while maintaining appropriate boundaries and a person-centered approach.
    • Assist participants in identifying and achieving short- and long-term goals by promoting healthy and adaptive coping strategies.
    • Support participants in developing practical skills and resources that promote sustained recovery, emotional wellness, and successful reintegration into daily life.
    • Serve as a liaison with family members, friends, partners, treatment providers, and community resources to establish a strong support network and ensure continuity of care.
    • Educate participants on behavioral health, recovery, wellness, healthy coping strategies, and daily living skills.
    • Collaborate closely with members of the multidisciplinary clinical team to address participant needs, monitor progress, and coordinate appropriate services.
    • Recognize and respond appropriately to participant concerns and crises, including assisting with crisis intervention when necessary and escalating concerns according to established protocols.
    • Participate in the implementation, presentation, evaluation, and revision of participant treatment plans.
    • Facilitate and/or present educational and therapeutic programming for participants.
    • Assist participants in developing individualized aftercare plans and connecting them with appropriate ongoing resources and services prior to completing the program.
    • Advocate for participants and help remove barriers to treatment, recovery, and continued wellness.
    • Maintain accurate and timely documentation in accordance with organizational standards and applicable requirements.
    • Uphold strict participant confidentiality in accordance with First Responder Wellness policies and procedures and all applicable local, state, and federal laws and regulations.
    • Perform other duties as assigned.

    Minimum Qualifications

    • Current license or registration with the California Board of Behavioral Sciences as one of the following: LMFT, LCSW, LPCC, AMFT, ACSW, APCC
    • Related counseling and/or case management experience in the substance use disorder treatment field.
    • Strong interpersonal, communication, organizational, and documentation skills.
    • Ability to work effectively as part of a multidisciplinary clinical team.
    • Ability to maintain professional boundaries and protect highly confidential participant information.
    • Demonstrated commitment to providing compassionate, respectful, and person-centered care.
