Job Description
A community-based health and human services organization on Long Island is seeking a new Case Manager to join their team in Suffolk County, managing a caseload of approximately 53 clients.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Combination of office and field-based work
Responsibilities:
- Conduct fieldwork throughout Suffolk County, including home visits and client accompaniment to medical appointments
- Coordinate services with treatment providers and advocate for clients with healthcare and treatment teams
- Schedule appointments, make referrals, and connect clients with community resources and benefits
- Complete intake assessments, psychosocial assessments, case notes, progress notes, and maintain client charts
- Support clients in understanding their medications, treatment plans, and available external services
Qualifications:
- Bachelor's degree in human services, social work, or a related field, or
- Associate's degree in a related field plus 1 year of case management experience, or
- High school diploma plus 2 years of case management experience
- Ability to conduct regular fieldwork throughout Suffolk County
- Personal vehicle and valid driver's license
