Full-time, temp to hire position in Philadelphia County. Monday through Friday, 9:00 a.m. to 5:00 p.m.; Thursdays, 10:30 a.m. to 6:30 p.m. Provide comprehensive case management services to families experiencing homelessness. Responsibilities include completing assessments, developing housing plans, providing supportive counseling, coordinating referrals, assisting with housing placement, maintaining timely documentation, collaborating with interdisciplinary teams and community providers, and utilizing trauma-informed practices to promote housing stability and family well-being.
