JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.
Prototypes is one of the nation's leading behavioral healthcare providers and is a "prototype" for such organizations. Prototypes Womens Center in Pomona is a 180-bed residential facility tailored specifically to women, including those who wish to involve their children in their journey to recovery. Our program aims to assist women in maintaining their progress in overcoming co-occurring disorders through a wide range of supportive services. Children can foster a secure attachment with their mothers in a secure setting, while also accessing mental health support and educational assistance. Our interdisciplinary team of professionals provides comprehensive treatment for substance use disorders and mental health, parenting, educational, and life skills courses. Treatment can range from 60 days to 6 months.
The Case Manager provides in-person support to clients by coordinating care both within HealthRIGHT 360 and with outside service providers. They work closely with clients to complete intake paperwork, conduct outcome assessments, and apply for benefits. As part of a multidisciplinary team, the Case Manager helps ensure effective treatment that aligns with HealthRIGHT 360's mission, values, and policies. Services are delivered flexibly in outpatient offices, residential facilities, or in the community, depending on client needs and program requirements. Therefore, reliable transportation is required.

QUALIFICATIONS
Education, Certification, Licensure, and Experience
Option I:
High school diploma or equivalent.
Substance Use Disorder Counselor registration from an accredited California agency (CCAPP, CAADE, CADTP).
Option III:
Bachelor's degree in Social Work, Psychology, or other related field from an accredited institution.
Case management experience.
Experience working with clients experiencing acute withdrawal from substances.
Experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery.
Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
Valid California Driver's License and access to registered and insured transportation.
