The Case Manager RN facilitates a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual and familys comprehensive health needs through communication and available resources to promote quality cost-effective outcomes.
Essential Functions:
- Conducts admission and continued stay according to established procedure and departmental standards.
- Establishes effective working relationship with medical and hospital staff to educate and coordinate usage of resources and services as evidenced by length of stay.
- Notifies director of potential or identified problem areas that concern length of stay or financial issues.
- Screens all cases for early identification of high risk" patients according to established protocols.
- Demonstrates patient and family involvement in the discharge planning process as indicated in the collaborative care notes.
- Assures patient care staff physicians are kept informed of discharge plans by providing a continued system of communication through documentation in the medical record according to established procedure.
- Ongoing appraisal of patient/family needs is evidenced by documentation.
- Assures completion of all discharge documentation referral agency forms and state required forms prior to the discharge of the patient.
- Keeps current community resources as demonstrated by providing accurate information on the resources to patients and family.
- Actively participates in Interdisciplinary rounding by appropriate selection of patients and discussing needs of patient with other departments.
- Follows regulatory compliance standards for IMMs choice letters and Certification forms.

Education:
- Associates Degree in Nursing
- Nursing License - Issued by Florida or Compact State
- Registered Nurse Certification
- Basic Life Support (BLS) from American Red Cross or American Heart Association
- National Institutes of Health Stroke Scale (NIHSS)
Work Experience:
- Two years of clinical experience as a registered nurse (RN) in an acute care setting.
