Case Manager-Registered Nurse

Registered Nurse Case Manager – Hospice | Pennsylvania | Full-Time

Overview

This is an excellent opportunity for a compassionate and experienced Registered Nurse Case Manager to join our client's hospice care team. In this role, you will provide essential patient care coordination, clinical leadership, and support to families navigating end‑of‑life care. You will work within a collaborative interdisciplinary team committed to delivering high‑quality, patient‑centered hospice services.

Required Qualifications

Education & Experience

• High School Diploma or GED
• 1+ year of Hospice experience
• 1+ year of Management or leadership experience
• 1+ year of experience in ANY of the following:
  • Home Health Care
  • Epic Home Health
  • Home Health

Licensure & Certifications

• Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Additional Requirements

• Comfortable with local travel across multiple counties
• Reliable transportation
• Willing to work required hours; no weekends unless desired

Key Responsibilities

• Manage and oversee patient care in hospice settings
• Develop, implement, and monitor individualized care plans
• Coordinate care with interdisciplinary healthcare teams
• Provide leadership, mentorship, and support to nursing staff
• Ensure compliance with hospice standards, policies, and state regulations
• Deliver compassionate, patient‑centered care to patients and families

Territory / Driving Expectations

• Travel radius: 15–25 miles surrounding the assigned community
• Daily local travel required across multiple counties

Schedule

• Monday – Friday | 8:00 AM – 6:00 PM
• No weekends unless desired
• 2 holidays per year

• Local travel required throughout multiple counties
• Mileage or travel reimbursement may apply depending on client policy

Pay Transparency Compensation varies based on location, experience, and performance. Some sites require disclosure of pay and benefits. This role may also be eligible for bonuses or incentives depending on business needs. Employees may receive health insurance, optional dental and vision coverage, life and disability insurance, retirement savings plans, paid holidays, and PTO or sick time. The estimated hourly rate for this position is $80K
