We provide integrated primary care and behavioral health treatment programs and assist with providing access to financial and educational resources where applicable.

Our dedicated Case Management team is trained to treat individuals with dignity and respect. A thorough intake is completed to assess immediate needs, eligibility for programs, and connection to area service providers. Through consistent follow-up, fostering and cultivating trust, and developing relationships, our clients receive the support network they need to better their livelihood.

RSP Case Manager (Rental Supplement Program)
Onsite, Full-time, Non-exempt position
Pay Rate: $24-26/hour
Schedule: M-F, 8am-4:30pm (40 hrs/week)

The RSP Case Manager provides intake/assessment, crisis intervention, essential/emergency case management, homelessness prevention, rapid re-housing, outreach and referral, and information services. This position will work collaboratively with the Program, Shelter, Residential, and Clinical: Medical Care, Social Work, PNP departments to ensure comprehensive services are provided to each individual in need.

Key Responsibilities

Manage all aspects of the Residential Support Program (RSP), including client eligibility, funding administration, and compliance.
Conduct client intakes, assessments, and application reviews to connect individuals and families with housing assistance and eviction prevention resources.
Monitor program funding, subsidy utilization, and client outcomes to ensure effective use of resources.
Maintain accurate client records and timely data entry in HMIS and other tracking systems.
Provide housing navigation, supportive counseling, and referrals to community resources that promote long-term housing stability.
Build and maintain partnerships with landlords, property owners, and community agencies to expand affordable housing opportunities.
Conduct housing inspections and assist clients in securing safe, stable housing placements.
Prepare required reports and documentation, including quarterly program reports and funding updates.
Collaborate with internal teams and external partners to coordinate services and support positive client outcomes.
Represent the agency professionally in community meetings, case conferences, and stakeholder engagements.

Qualifications

Bachelor’s degree (or pursuing a bachelor’s degree) in human services or related field.
Experience providing services to the homeless or related human services necessary.
Knowledge of agencies, programs, and services in the community.
Demonstrated ability to effectively engage and support individuals experiencing homelessness, mental illness, substance use challenges, and those who may be difficult to engage.
Valid driver’s license (for one + years), ability to safely transport clients as needed to support access to services and housing stability.
Strong computer proficiency, including experience with data entry, tracking systems, and working with financial or program-related data.
Ability to manage a full-time schedule in a fast-paced environment while maintaining accuracy, organization, and attention to detail.
Skilled in conflict resolution, with the ability to navigate challenging situations professionally and achieve constructive outcomes.
Proven ability to prioritize tasks and meet deadlines, including flexibility to adjust work hours when necessary to meet program goals.
Commitment to professionalism, consistently representing the agency with integrity, respect, and cultural sensitivity in all interactions.
Knowledge of eligibility criteria and application processes for mainstream/entitlement programs helpful.
