Case Manager -- Residential Housing - 1 Day Remote

Contract to Hire

Mitchell Martin Healthcare is partnering with a leading nonprofit human services organization to hire a Case Manager to support residents in a community-based residential setting in New York City.

As a Case Manager, you'll build meaningful relationships with residents, advocate for their needs, connect them with important resources, and help them navigate healthcare and social services. You'll also have opportunities to lead groups, participate in activities and outings, and support residents in the community.

This role is a great fit for someone who is compassionate, organized, resourceful, and comfortable working with individuals who may have complex social, behavioral health, or healthcare needs.

Schedule

Standard schedules include:

  • 8:00 AM – 4:00 PM
  • 8:30 AM – 4:30 PM
  • 9:00 AM – 5:00 PM

Additional schedule requirements:

  • One late day per week, generally extending the regular schedule by one hour
    • Example: 9:00 AM–5:00 PM → 10:00 AM–6:00 PM
    • Example: 8:30 AM–4:30 PM → 9:30 AM–5:30 PM
  • Saturday coverage approximately once per month
  • Employees who work a Saturday receive a weekday comp day during the same week
  • After one month, employees become eligible for one hybrid workday per week, based on program needs and approval

What You'll Do

  • Manage an assigned caseload and meet with residents regularly to understand their needs and goals.
  • Build supportive, professional relationships with residents and advocate on their behalf.
  • Connect residents with healthcare, behavioral health, housing, social services, benefits, and community resources.
  • Coordinate and accompany residents to medical, psychiatric, and other appointments as needed.
  • Help residents navigate healthcare and social service systems.
  • Develop and maintain individualized service plans.
  • Monitor resident progress and help identify barriers to achieving their goals.
  • Facilitate and participate in groups, activities, outings, and community events.
  • Lead at least two ongoing resident groups or activities.
  • Provide support during crisis situations and connect residents with appropriate emergency resources when needed.
  • Conduct regular resident/unit check-ins and assist with identified needs.
  • Maintain accurate case notes, assessments, service plans, and other required documentation.
  • Keep client records complete, accurate, and up to date.
  • Maintain confidentiality and follow HIPAA and privacy requirements.
  • Collaborate with healthcare providers, social service agencies, community organizations, and internal team members.
  • Support residents in developing greater independence, wellness, and community involvement.

Qualifications

Required:

  • Bachelor's degree, preferably in Social Work, Human Services, Psychology, Sociology, or a related field
  • Minimum 2 years of experience in human services, case management, social services, behavioral health, residential services, or a related field

  • Working with individuals experiencing homelessness
  • Behavioral health or mental health services
  • Substance use services
  • HIV/AIDS services
  • Benefits and entitlements
  • Supportive housing or residential programs
  • Community-based social services
