Case Manager

As a leading Residential Treatment Center in the Greater Richmond area, Hallmark Youthcare treats adolescents with emotional and behavioral issues triggered by trauma. Hallmark Youthcare is currently accepting applications for 2 Case Manager's. 

Duties include but not limited to:

• Coordinates updates to individual services plans and communicates progress to family and/or custodians.
• Supports therapist with discharge planning and confirmation of after-care services prior to discharge

The primary treatment methodology use by the therapists is Dialectical Behavioral Treatment (DBT), familiarity with the fundamental treatment constructs is preferred. If you are highly organized and methodical about coordinating patient-centered services we welcome an opportunity to speak to you.

Hourly: $26-29, based on experience and education.

Experience:

• Bachelor’s in Social Work, with LMSW credentialing (preferred)
  • Supervision available
• Case Management (1 year)
