The Case Manager is an experienced professional who provides support, guidance, and connections for students experiencing distress at UCR. Case managers are licensed clinicians who work within a FERPA environment to provide non-clinical case management services. Case Managers use their applied professional knowledge and experience to independently performs the full range of case management responsibilities in support of working with a diverse student population across a range of academic and personal challenges to collaboratively develop goals, interventions and emphasizes identifying resources to address students' medical, psychological, psychiatric, social, financial and academic needs. Case Managers also serve as a resource for faculty, staff and families in support of student well-being. This role will specifically provide direct and indirect support to Student Health Services department and will spend dedicated time to provide patient support and advocacy. The full salary range for the Case Manager is $90,034.56 - $119,141.28 annually. However, the expected pay scale for this position is up to $100,000.00 annually. We base salary offers on a variety of considerations, such as education, licensure and certifications, experience, and other business and organizational needs. Applicants must have current work authorization when accepting a UCR staff position. Currently, we are unable to sponsor or take over sponsorship of an employment Visa for staff. Federal, state, or local government directives may impose additional requirements.

Case Management, Crisis Intervention, Resources linkage, Support and Advocacy: Provides a broad range of non-clinical case management support to UCR students with an emphasis on identifying and coordinating resources to address students' psychological, social, financial, and academic needs. Utilizes clinical expertise to independently assess relevant student history in support of formulating strategies for students leading to improvement and resolution of barriers to success. Assists students in problem solving strategies and navigating complex University processes. Provides crisis management and follow-up support for students who have psychiatric hospitalization. Develops and communicates action plans/goal setting, provides crisis intervention/management and supports campus emergency services as required. Other general case management duties as assigned or required.Administrative Duties, Case Management Follow Up and Consultation: Administrative duties: Case Managers provide administrative support for the department by answering the department phone line, returning calls, scheduling students, managing/responding to department emails and appointment requests, and managing Advocate Public Care Reports. Some general administrative support will be during pre-scheduled consultation hours. Case management follow-up and consultation time: Case Managers collaborate and consult faculty, staff and other community members regarding students in distress and their ongoing care. Consult regularly with key campus partners for students with high risk mental and physical health conditions and assist with referrals to community providers. Meets regularly with community providers, agencies, and services, to ensure effective response to student needs, and a smooth transition of care to community, as needed. Documentation: Case Managers use administrative time to prepare for student intake appointments; follow-up and respond to student phone calls, emails; and complete any needed documentation, such as completing appointment and contact documentation and letters of support on behalf of students based on departmental guidelines. Case Managers keep detailed records of consultation, initial assessments, progress notes, and follow-up in a timely matter based on policies and procedures and in alignment with profession and legal standards of practice.Outreach, Presentations, Workshops: Case Managers collaborate often with campus partners to provide departmental trainings or presentations for faculty, staff, students and families on services provided by the case management department. Case Managers may provide specific trainings for preventative programs or workshops for suicide prevention, mental health education awareness, working with distressed students, and/or any other HWS needs, as requested. Professional development: Case Managers remain up to date on best practices by participating in relevant conferences, obtaining continuing education units, and renewal of licensure.Other duties assigned: These may include provision of training and support to department's student staff, which may include training and monitoring student workers' tasks and duties. Provides guidance and educates student workers on day-to-day Case Management business, such as, answering phones, scheduling student appointments, supporting social media efforts and/or creating flyers/posters; or any other duties assigned by department.
