Founded in 1921, Straub Medical Center includes a 159-bed hospital in Honolulu, a network of neighborhood clinics and a visiting specialist program that reaches throughout the state of Hawaii. With over 200 physicians who are leaders in their fields, Straub provides its patients with diagnoses and treatments for more than 32 different medical specialties, including bone and joint, heart, cancer, endocrinology/diabetes, family medicine, gastroenterology, geriatric medicine, internal medicine, vascular and urology. Straub is home to the Pacific Region's only multidisciplinary burn treatment center. The hospital consistently brings new technologies and innovative medical practices to Hawaii, such as minimally invasive cardiac surgery and total joint replacement.

The Case Management team puts great emphasis on helping each patient transition their health care treatment with an appropriate set of support tools and information. Our caring, professional staff provides timely medical and discharge management to meet clinical and service goals, and to provide an effective, appropriate continuum of care. We want to ensure that our patients receive the attention they need beyond the hospital walls, especially when they require care from several different organizations and other combinations of medical, financial or social interventions.

As the Registered Nurse Case Manager, you would ensure the considerate and confidential management of unique and high-risk patient cases by performing case management through direct referrals and reviews of medical records. This would include determining the appropriateness and medical necessity of admissions, continued hospital stay, and the use of ancillary services. You would also perform treatment plan assessments, discharge needs interviews, planning/coordination for high risk cases, and participate in organizational quality improvement activities. We are looking for an individual who is perceptive to patient needs, with outstanding organizational and communication skills and a commitment to delivering the highest quality health care to Hawai'i's people.

Minimum Qualifications: Current Hawai'i Registered Nurse license. Bachelors degree and/or equivalent combination of education, training, or work experience. Three (3) years clinical experience in acute care, community or long term care, or related field.

Preferred Qualifications: Current Case Management or Utilization Management certification.
