Kapiolani Medical Center for Women & Children is Hawaii's only maternity, newborn and pediatric specialty hospital. It is well recognized as Hawaii's leader in the care of women, infants and children. With 253 beds, the not-for-profit hospital delivers 6,000 babies a year, and is also a medical teaching and research facility. Specialty services for patients throughout Hawaii and the Pacific Region include intensive care for infants and children, 24-hour emergency pediatric and adult care, critical care air transport and high-risk perinatal care. Over 1,400 employees and more than 700 physicians provide specialty care at Kapiolani. The hospital is home to the Kapiolani Women's Center and the Women's Cancer Center, and offers numerous community programs and services, such as specialty pediatric clinics, the Kapiolani Child Protection Center and the Sex Abuse Treatment Center.
The Case Management team puts great emphasis on helping each patient transit their health care treatment with an appropriate set of support tools and information. Our caring, professional staff provides timely medical and discharge management to meet clinical and service goals, and to provide an effective, appropriate continuum of care. We want to ensure that our patients receive the attention they need, especially when they require care from several different services and other combinations of medical, financial or social intervention.
As the RN Case Manager, you will ensure the considerate and confidential management of unique and high-risk patient cases by performing case management through direct referrals and reviews of medical records. This includes determining the appropriateness and medical necessity of admissions, continued hospital stay, and the use of ancillary services. You will also perform treatment plan assessments, discharge needs interviews, planning/coordination for high risk cases, and participate in organizational quality improvement activities. We are looking for someone perceptive to patient needs, with outstanding organizational and communication skills and a commitment to delivering the highest quality health care to Hawai'i's people.

Minimum Qualifications: Current Hawai'i Registered Nurse license. Bachelors degree and/or equivalent combination of education, training and experience. Three (3) years clinical experience in acute care, community or long term care, or related field.
