RN - Case Manager – Hermiston,OR

Local Pay: $66.38/hour

Travel Package:

Travel Rate (based on 40 hours): Hourly $39.53, Stipend (Lodging $770.00 + M&I $476.00) = $1,246.00, 

Total Comp:$2,827.20

Travel Eligibility: Candidates must live at least 50+ miles from the facility to qualify for Travel Pay.

Shift Details:

  • Days, 4x10's, with weekend and holiday rotation
RN Case Manager  Duties & Responsibilities:

  • Assesses patient needs and develops, reviews, and updates comprehensive care plans.

  • Coordinates care transitions, referrals, and continuity of care across healthcare settings.

  • Evaluates medical necessity, monitors length of stay, and identifies discharge barriers.

  • Coordinates insurance authorizations and ensures appropriate use of healthcare resources.

  • Advocates for patients and educates patients and families on treatment and self-care.

  • Connects patients with community resources and addresses financial, social, and logistical barriers.

  • Maintains accurate case management documentation and tracks patient outcomes.

  • Collaborates with physicians, therapists, social workers, specialists, and other healthcare professionals.

  • Participates in interdisciplinary care conferences and coordinates care plan implementation.

  • Maintains patient confidentiality and communicates with empathy, professionalism, and cultural sensitivity.

  • Supports hospital policies, required training, and quality improvement initiatives.

  • Performs other related duties as assigned.

Minimum Qualifications:

  • Current, valid, and unrestricted OR license/certification in good standing, as required for the position.

  • Graduate of an accredited school of medicine, social work, nursing or related medical occupation.

  • Previous clinical experience in an acute care setting, or home health setting assessing skilled care needs and equipment and health maintenance resource needs.

  • Previous Discharge Planning and/or Case Management experience.

  • CRASE training. If travel assignment extends beyond a 6-month period, CRASE training will be required.

  • Additional certifications may be required

  • Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

  • Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
