Position Summary:
Manage a designated caseload, to coordinate and complete timely assessments, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Collaborate with medical staff, unit managers, and all professional care giver disciplines to plan and coordinate the plan of care with patients and families throughout the acute continuum of care and into post-acute settings. Ensure optimum utilization of the patient's and the Health System's resources and perform these duties within the requirements of CMS and other external review agencies. Respond to Director/Manager as required.

Excellent communication and interpersonal skills
Ability to assess patient needs and develop care plans
Knowledge of health care systems and community resources
Experience in case management or a related field
Understanding of medical terminology and documentation
