A Case Manager RN opportunity is available at a 221-bed acute care hospital and Level II Trauma Center in Sanford, FL. The hospital offers a full-service cardiovascular program — including open-heart surgery, interventional cardiology, electrophysiology, cardiac rehabilitation, and comprehensive diagnostic services — alongside orthopedic and spine care, robotic surgery, acute inpatient medical rehabilitation, and complete imaging services.

The role. Reporting to the Director of Case Management, this position covers all inpatient hospital units and focuses on guiding patients and families through every step of their care journey. The Case Manager conducts psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs clearly across the interdisciplinary team. You will develop and implement individualized treatment plans that reflect each patient's strengths, recovery goals, and personal circumstances, while coordinating education for patients, families, and caregivers to encourage active participation in treatment planning and family conferences. Close collaboration with the Rehab Program Director, Facility Case Management Director, and the broader care team ensures seamless, patient-centered transitions across the continuum of care.

What we're looking for.
  • 2+ years of recent (within the last year) acute care Case Management RN experience
  • (APRN) Advanced Practice Registered Nurse, or (RN) Registered Nurse
  • Associate Degree, or Registered Nurse Diploma

Schedule. Days with rotating weekends (every other Saturday, possibly every third once fully staffed); shift times vary across Monday–Friday and include 8:00 a.m.–4:30 p.m., 9:00 a.m.–5:30 p.m., 10:00 a.m.–6:30 p.m., and 11:00 a.m.–7:30 p.m.

Compensation. $36.96 - $44.00/hr plus Sign-on bonus available
