POSITION TITLE:  Case Manager - THP                                                             FLSA: Non-Exempt

SUPERVISOR: Director of Residential Services                                               Job Code: NA

Salary Range : $23 to $25 an Hour 

The Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program (THP) provides direct support and intensive case management services to assist Transitional Aged Youth (TAY) ages 16 to 21 years, with meeting individualized goals while connecting them with resources in the community.  The position is collaborative and coordinates care on an interdisciplinary team.

QUALIFICATIONS:

Required:

  1. Bachelor's Degree in one of the following areas: Marriage, Family and Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Clinical Psychology, Educational Psychology, Education with an emphasis on Counseling, Social Work or Social Welfare, an area that includes counseling or psychotherapy content required for licensure as a Licensed Professional Clinical Counselor
    1. OR, a Bachelor’s Degree in one of the following areas: Human Services or Behavioral Science, AND two (2) years of experience in a public or private child welfare social services setting, with specific experience working with TAY 16 – 24 years of age.
    2. OR, CCL approval as an Excepted Case Manager prior to start date
  1. Experience working with the TAY population
  2. Knowledge of Microsoft Offices applications

 

RESPONSIBILITIES INCLUDE (but not limited to):

  1. Program Implementation and Direct Client Services:
  1. Utilize a variety of approaches to establish and maintain a relationship with youth
  2. Maintain individual contact with each youth at weekly
  1. Support youth
  2. Participate in relevant team meetings such as; Child Family Team meetings, TDMs, and emancipation conferences 
  3. THP Program Statement
    1. Maintain knowledge of THP Program Statement
    2. Implement services per the THP Program Statement
  4. Attend required training to maintain knowledge and certification needed for the performance of the role.
  5. Staff will be required to cover shifts as needed and on an emergency basis.
  1. Compliance and Reporting:
  1. Responsible for completing reports, notes and Medi-Cal paperwork in a timely manner, as applicable
  2. Responsible for inputting data into the client database within 2 days of change or receipt of information
  3. Ensure records, documents and paperwork is up-to-date and maintained on-site
  4. Maintain knowledge of CCL Regulations

       3. Program Productivity:

  1. Case Managers are expected to meet productivity standards, as applicable.  
  2. Maintain accurate and billable case notes per Medi-Cal requirements, as applicable.
  3. Ensure 75% of youth graduations
  4. Ensure all clients are evaluated and assessed with proper documentation and established assessments
  1. Program Ambassador:
  1. Serve as community liaison and agency representative
  2. Interface with County Child Welfare and Probation staff on all facets of case coordination
  3. Research and develop community resources for youth
