Case Manager I (Full-Time)

Position Summary

The Case Manager I plays a vital role in supporting patients through comprehensive case management services. This position focuses on helping individuals obtain income, insurance, housing, and access to medical and behavioral health treatment. The Case Manager I provides supportive services to patients with co-occurring mental health, substance use, and medical needs—empowering them to improve functioning, build skills, and achieve goals.

This role includes both office-based and community-based work, including regular field visits and transportation of patients to appointments and essential services.

Key Responsibilities

• Conduct one-on-one intake assessments to identify patient needs.
• Maintain and manage a caseload of approximately 50 patients.
• Document progress notes daily in the electronic health records (EHR) system.
• Provide patient-centered support and track progress toward measurable outcomes.
• Perform approximately 30% in-office and 70% community-based duties.
• Maintain 80% daily utilization of billable patient services.
• Transport patients to appointments and community resources as needed.
• Participate in multidisciplinary care coordination meetings.
• Advocate for patients in securing identification, benefits, housing, and entitlements.
• Develop collaborative SMART treatment plans with patients.
• Participate in rotating after-hours on-call duties (including weekends).
• Complete required trainings, certifications, and renewals.
• Attend team and departmental meetings and support program operations as needed.
• Perform other related duties as assigned.

Required Qualifications

• High School Diploma or GED required; Associate’s Degree preferred.
• Lived experience working with populations such as unhoused individuals, those with serious mental illness, substance use disorders, or chronic conditions.
• Ability to work collaboratively across diverse teams and maintain appropriate boundaries.
• Strong verbal and written communication skills along with effective listening skills.
• Ability to work in a fast-paced environment with strong organizational skills.
• Commitment to serving underserved communities.
• Proficiency in Microsoft Word, Excel, and Outlook.
• Current CPR and First Aid certification (or willingness to obtain within 90 days).
• Ability to maintain confidentiality and adhere to HIPAA requirements.
• Willingness to follow infection control procedures.
• Valid driver’s license, insurable driving record, and willingness to use a personal vehicle to transport patients.
• Flexibility to work in varying locations and adapt to changing program needs.

Work Environment & Physical Requirements

The role involves typical office activities as well as field work, including interactions with individuals who may be experiencing crisis, mental health challenges, substance use, or limited access to personal care. Occasional exposure to bodily fluids or hazardous materials may occur.

This position requires:

• Frequent sitting, walking, standing, and reaching
• Ability to lift or move up to 20 lbs
• Adequate vision, hearing, and speech for in-person and phone communication
