Program Description

Felton’s Senior Division specializes in a full continuum of mental health services for older adults in outpatient clinics, the community, and supportive housing sites, as well as programs in employment training, economic security, case management, advocacy, and social connection and inclusion.

 

Felton’s Knox Supportive Services Program provides onsite case management, wellness support services, and behavioral health services to assist tenants with a LOSP subsidy at The Knox. Services aim to support tenants in maintaining stable housing and improving access to resources. Supported tenants were formerly unhoused adults with co-occurring mental health, substance abuse, medical problems, and limited experience living independently. Support services include case management and behavioral health counseling, referral to and coordination with primary medical care, substance abuse and psychiatric treatment referrals, benefits counseling and client advocacy, meal programs, health education, community building, tenant organizing, and all other case management functions. Felton is committed to implementing evidence-based practices and behavioral health research within its community-based mental health services. 

 

Job Description

Under supervision of the Program Director, the case manager participates in a multidisciplinary team, including external and internal agency team members consisting of mental health clinicians, primary care providers, nurses, peer specialists, interns, and psychiatric medication providers. The program serves 30 tenants at The Knox. The tenants occupying the additional units in the building will not be part of the case manager’s caseload. The caseload is capped at a lower level to allow for intensive services to stabilize individuals who experience acute symptoms and related risk factors. The case manager evaluates needs of clients and works in partnership with a mental health clinician and program director to support the client with coordination of care. Services may include crisis intervention, connection to community resources, client advocacy, and engagement strategies. The case manager maintains accurate, detailed records of client contacts and incident reports as required by Felton, HSH, and county and state regulations. Additional duties are assigned as needed. This position requires the ability to perform the essential functions of the role with or without reasonable accommodation. These functions may include moving and positioning items weighing up to 40 pounds, bending, reaching, stooping, moving throughout the work environment as needed, and responding promptly and safely to urgent or emergency situations, including moving quickly to provide assistance when necessary.

 

 Specific Responsibilities

• Work closely with interdisciplinary team.
• Communicate effectively with all team members internally and with partnering agencies.
• Perform intakes and service plan development for adults with a history of being unhoused and other psychiatric or medical conditions or disabilities.
• Provide case management services, which could include connection to primary care, coordination with other providers and community resources, housing stabilization services, etc.
• Maintain records of each client contact or service in accordance with mandated standards of HSH and Felton. Meet requirements of timely access for client care and documentation.
• Participate in team meetings and case conferences through presentation of cases, discussion of issues and exchange of information and ideas.
• Provide crisis intervention to prevent hospitalization and collaborate with clinician as needed.
• Respond to emergency situations as they arise.
• Address specific housing needs via training, coaching, or community resource referrals to support clients in maintaining stable housing.
• Commit to ongoing training, including developing specializations in evidence-based practices.
• Use the ONE system to maintain client records related to housing per HSH standards.
• Deliver exceptional customer service to clients, community partners, and team.
• Other duties and responsibilities as assigned. 

Minimum Qualifications                                                                                                   

• A minimum 2 years of case management for at-risk populations, including co-occurring disorders.
• Experience working with unhoused or formerly unhoused community members, with a strong grasp on housing needs and potential barriers to maintaining stable housing.
• Experience managing crisis situations, skilled at crisis response, crisis de-escalation, and risk mitigation.
• Passionate about working with adults who may be isolated, have mental health issues, have cognitive impairments, substance abuse issues, or other needs that are not being met.
• Demonstrated outreach, rapport building, and engagement experience.
• Ability to travel in SF and work in office, community, home, and hospital settings.
• Strong skills in working independently and also as a team member.
• Strong organizational, communication, interpersonal, and time management skills and ability to prioritize projects and follow through.
• Problem solving skills: able to discern when to consult, when to make independent decisions, and able to offer suggestions for solutions.
• Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds and who often are underserved by traditional care models.
• A strong understanding of harm reduction in supportive housing.
• Actively practices cultural humility and promotes a warm and welcoming environment for culturally and gender diverse individuals.
• Openness to learn, consult, and receive constructive feedback.
• Available to work business hours, Monday through Friday, predominantly in field/office settings.
• Excellent communication skills both in written and verbal formats.
• Demonstrated computer skills (Microsoft Office, Mac and PC OS) and ability to learn electronic documentation systems.
• Previous experience leading activities and groups preferred.
• Driver’s license preferred.
• Lived experience with mental health services/peer certified/trained preferred.

FLSA Status: Exempt
Salary Range: $70,000 - $80,000 Annually (DOE)

This position is union-represented by SEIU Local 1021 and is subject to the applicable collective bargaining agreement.
