Middlesex County is seeking a dedicated and compassionate Case Manager to join our Special Child Health Services (SCHS) team. This position plays a vital role in supporting Children and Youth with Special Health Care Needs (CYSHCN) and their families by coordinating services that promote optimal health, development, independence, and overall well-being.
The ideal candidate is a licensed Registered Nurse or Social Worker with experience in pediatric case management and a strong commitment to family-centered care. Bilingual proficiency in Spanish is preferred.
The Case Manager will work collaboratively with children, families, health care providers, educators, therapists, community organizations, and other service professionals to identify needs, coordinate resources, develop individualized service plans, and monitor progress toward established goals.
